Contact the provider and see if she is aware of this.  Perhaps she had some reason to want to have her reviews pulled, or she ran afoul of TER for something.

In any case, only she can deal with TER to get her reviews back up.  If she mananges to do that, then you can post a review.

I had a Boston local provider saved in my Favs and saw her too today. But when I got on to submit a review, it says the provider info is not available any more...?
