This question is really dumb.  If the lady screened the guy then she knows who he is before he gets to her door.  In fact, she would have decided as soon as she heard the name on the phone or in the initial email. This would not have gone very far.

I can think of other circumstances where this mix-up might happen but not with any amount of due diligence on the hobbyist or lady's part.

Ladies, have you ever had a potential client come to the house and as you look out the peephole of the door realize you know him in real life? What did you do, not answer the door?

If he passed screening, there is no reason not to see the fellow.

you look out the peephole and he's your cousin?  or your college professor?  or your kid's teacher or coach?  or a dozen other extremely akward scenarios?

Lots of good reasons not to see gentleman at your home.......

So you're a hobbiest on an out of town trip. You contact a local agency and book an out call. When the door rings you answer and there is your daughter who attends college in that town......or granddaughter, or niece or ex wife....  or you book a dom session and your old school teacher is there....Lol    I mean the possibilities are endless......

Yes....it happens!!!    I once booked a session to find it was a woman in my office.   interesting.....

I have not met family, but I have seen ex-girlfriends and college/business acquaintances this way.
Life Happens.

There was a young woman once who (in the after-glow conversation) said "You're from XXXXXXXX? So is my mom. Her maiden name was KKK YYYYY." Yep, I knew her mom from elementary through high school and even kissed her once during a slow dance in junior high. Though this woman was born well after my high school days with her mother, it still was a bit awkward for a few minutes as that 'small world' feeling passed by. I think she left the Hobby a few years ago.

Anyone else care to share something similar?

Never happened to me but agreed. As well as many of us screen, we would or SHOULD know long before the gent showed up at the door.

However...

As a few of the gents mentioned there are scenarios where you haven't seen that person in ages or the guy possess a very common name and shows up at the door only to realize he knows the girl somehow someway that he has booked his date with.  Man, if I could only be a fly on the wall!
