I suspect that in many cases it is also the real answer. Let's face it, this is a business, and an awfully intimate one at that. A provider, if a professional, will take on all comers, within reason. As I understand it, missionary even being possible is a fairly recent development in evolutionary term. Our ancestors knew doggie and that was it. So, maybe it still feels more natural?
