Sometimes it's an issue with the ad link. Sometimes it just needs another staff member to look it over. Could be a lot of things. You can always contact them and ask. It may be something where you can send them a bit more info, and then it gets approved.

or the provider gets another review to help prove the review is not a fake. This sometimes occurs when the provider and the reviewer are new to TER and/or have a limited posting history.

If the OP asked this question because one of his reviews was deferred, your explanation is certainly not the reason in his case, since he has 92 reviews. :)

I got my answer and pulled the review for now.  It had to do with my providing a review from a perspective of several sessions over the last two months rather than being more specific about a single experience.  I asked moderators to pull as I have decided not to kiss and tell in this case.
