My first reaction was I prefer Katz's, but then I realized you can only smoke meat there, not cigars. Sorry, NY joke.

Actually, the two most accessable cigar bars are Carnegie (the bar not the deli) and Club Macanudo. The Bar and Books on 1st Avenue requires you to smoke in a seperate back room, and the Grand Havana Room is a membership club.
