Usually I have know this to mean the additional is Greek. (Though not every porn has anal sex in it) However, it could also include video and toys. Plus, I think some consider PSE just the act - no chatting or getting to know each other. Therefore there is more "action" time giving reason for the higher rate.
But I am no expert, nor have I ever offered PSE, and I no longer use the term GFE. The terms are so vague and subjective.

I'm not sure what circles those are, I have never heard that.
GFE is more of a romance-induced session with seduction and passion. PSE is more naughty. Not passionate and romantic, just hard core fulfillment. PSE sometimes includes things that GFE doesn't. But each lady sets her own boundries. Afterall, what is PSE to one is GFE to another. It's best just to ask.
While we're at it, GFE in fact doesn't necessarily mean the emotional stuff referenced below. I know in theory it does.
