So, I clicked it and sent in a message that the reviewer had reviewed twice under his own name and the third review was quite possibly the same person.

Please - when you see issues like that, hit that problem report button.  It helps all of us by adding extra eyes and brains to the huge task of trying to keep the review database clean.

With multiple people approving reviews, and hundreds of reviews to approve each day, these things can slip through the cracks.

And, there can be exceptions to the one review per  guy per provider.  If a gentleman sees a lady at some time, and then she provides either a new service, or her service level drops significantly, a new review will be allowed.
