The Policy "if you go over the time limit" should have been made known to you - upfront. No one like surprises - had you known in advance perhaps you would have spent your time better planning your "nut".

If I understand correctly...your time was up and she stayed the full hour and said she had to go, despite you not getting to cum. That's not an upsell. I'm going to present a few perspectives which may rub up against your model of the world here.
A) You failed to state how long you contracted for. These things are important since it affects outcomes and it's hard to give you an informed answer.
Yesterday, I had a new client who wanted to see an associate and he asked for an 1.5 hours. During the course of the conversation he asked about going to dinner. Excuse me ? I was obligated to explain to him, that anything less that three hours with a restaurant within a mile of the hotel is a formula for failure. Stop and think about the number of variables out of your control when you go to a restaurant . Had I not known my business, had I just booked the call, I and my associate would have to deal with a client with unrealized expectations. As I teach my associates, the time to take care of a problem, is before you have one.
Without knowing how much time you asked for, can't really answer your question.
B) With encounters after 10 PM, you can into the possibility of her wanting to "get her done" and get her ass home. Especially, if she lives more than an hour away from where you are. You did infer the call ended at 1 AM therefore the " I want to get my ass home factor" is not an unreasonable premise.
I'm going to say the premise of your measurement is wrong. "Fault" is not the correct measurement here. It sounds more like your expectations where not properly managed. Yes, you bare some of the "responsibility" for time management. However the girl also "perhaps" should have been professional enough to "redirect" things along especially if the time you contracted for was not realistic to the situation or your personal pattern (flow).
If this is your internal framework and measurement system, then congratulations you just set yourself up for emotional / mental failure. The only person who see this as a battle is you. Your thought process has taken this situation and made it in to a win -lose premise, completely ignoring any and all positives of the encounter.
You got the girl you asked for, she showed up, she gave the time you paid for, you didn't get beat, ripped off or ended up calling your lawyer.
As for what Newto1000 and DCdaytimer had said, with regret I to must agree, what they have present to you could also be true.

Always keep this in mind, you are PAYING her for her TIME. What you do between those times is your business. I pay to fuck, and that is all I want to do. If she wants to talk later, then that's fine to.
Another thing. When you are paying someone for their time, that its a contract. The contract is the business end. However, on a personal end, do you think she wants to see you if its not a contract? No, she doesn't want to see you on a personal end.
Some of these guys looks like Shriek, are socially inept, actually looking for a girlfriend, and don't know where to draw the line between business and hobby. She has to deal with that. I myself can't imagine sleeping with Shriek.
So, unless you look handsome and attractive, and you feel that she is connecting with you, then you should always be mindful of the time. If you look like Shriek, then go in, get laid first, then talk later. Don't waste your time talking, drinking wine, or anything that is not sexually related. Unless, you are paying her for more time. If you paid her for 3 hours, then wine is appropriate. If you pay her for 1 hour, then you need to get your nut in first and talk afterwards.
Indy or not free extra time just does not compute UNLESS she wants to give it to you. Her saying her agency would dock her pay does sound fishy though. Never worked for one but that does sound odd.

If you see agency girls regularly, you should know most of them are clock watchers. They have to check in when the session starts and check out when the session ends. Don't you hear the music or the phone ring when your time is up? If they go over time, they'd better have a good explanation to the agency. They have to be punctual or their schedules would screw up with disgruntled clients waiting impatiently in the parking lot.
You would have better luck with an indie but not always. A lot of times, I go overtime (usually no more than 15-20 minutes) with them without any problems, especially if they are local and have no plans after my session. But last week, I was with a well-reviewed touring provider for a one-hour sessison. I was getting very close to finish and she could sense that (I also told her I was getting close). Then all of a sudden, she asked me if I wanted to extend. I looked at the clock, and noticed that my time was almost up. I decided to extend and came 8 minutes later (since she watched the clock, I figured I might as well). We spent the next 20 minutes or so just shooting the breeze. I couldn't help but feeling a bit of resentment like you did. I decided not to see her again (it was my second visit with her)
