Based on their website they don't require any type of screening. They want you to fill out a form with your "full name" and race. I wouldn't be looking to give them detailed personal information but what they're asking for is basically nothing especially considering that you can just make up a bogus name.
I would be very surprised if anyone new was approved just on the basis of the form. I think that is probably a first screen to see if they even want to ask for additional requirements, like provider or other agency references.
