1. The provider in question is retiring  and does not her information on the board anymore.
2. She could be reinventing herself because of a stalker or personal reasons and do not want the reviews attached to her new persona.
3. Her review could have been going down hill and she does not want people to see that since it will hurt her business.
4. She could have gotten some bad review, which she can't get pulled, so she removed her whole profile from TER.

I'm sure there are quite a few more reason for this to happen, but these are my guesses.
