Over the past 4 weeks I've experienced 6 instances of well reviewed independents or agency girls flaking on me -- either cancelling at the last minute or simply standing me up. WTF?

I won't argue your statement that YOU never cancel, but to claim that only the men pull this kind of thing is self-serving bullshit that NO ONE here will believe. Next time you want to tell a lie, try telling a believable one.

One told me she needed to eat dinner when I booked an appointment, I decided not to book. Another made me wait for a hour, found out later on she was eating dinner. I could have seen another provider instead.
I’ve had ~ 6 appointsments over the last 2 months and no body has flaked. One lady, who’s is almost always late, was a tad late but continues to be worth it.
Feel free to PM who. I'm guessing AME and JM if I had to venture a guess on agencies. I haven't had any issues, but I haven't dated any true indies for awhile. The last visiting indy flaked at the very last minute, and I mean literally the last minute. The other gave confirmation, then booker said she was stuck in traffic.
It depends on the provider. I've only had one cancel on me, only to find out she never even made it to town. Her excuse was too many people cancelled on her. Problem was she told me this the day of.
