About 2 days later I just got a note saying I was approved for an appointment, BUT that I would need to bring my Drivers License to prove identity. That's a deal breaker for me I am afraid...

Not sure why that would be a deal breaker -- but everyone has a right to their own standards. A number number of providers have requested that on the very first visit to verify that I'm the person they screened. In one case I'm not even sure the girl could actually read the thing.
