I have the same fetish and always inquire prior to a visit if they are a smoker and all have been willing to smoke for me during the act or to give a smokey bj.  My goal is to find a provider that shares the same fetish.  Just watched She Male Road Trip 8 and there was a great smoking scene with Sara Sin (search TS reviews in LA and also search on "Smokes during session").  My guess is she has the fetish.
