Really desperate for help in treating PE (long)

Hello Love Goddess.  I know this subject has come to you several times before but I'm at my wit's end trying to determine a good course of action with my own PE situation. Forgive this long post but let me detail my own 'history' first.

My history of trying to delay my own orgasm (ejaculation) is very long (my entire sexual life).  At first I tried the usual desensitizing creams, sprays, stop/start, squeeze, etc with no success.  Finally in 2002 I went to see a 'specialist' in a male clinic while I lived in London.  Though I didn't know it at the time he prescribed for me a daily regimen of Seroxat (paroxetine - Paxil here in the US) and Yohimbine.  This 'treatment' was very effective in delaying orgasm (and heightening the experience) but had side effects that were very difficult to deal with. Essentially, I was feeling 'agitated' most of the time and if I tried to exert myself physically (other than for sex) I felt like my heart was racing out of control and I became very jittery.  The clinic then reduced the dosage of the YH.  I stayed on this treatment for 3 or 4 months while I was in England but when I returned to the US I just couldn't bear the side effects so stopped taking everything.

A couple of years ago I began researching the use of SSRI medications (anti-depressants) to delay ejaculation.  My research led to a drug called Trazodone.  I tried this with poor success (little delay and a different side effect - I would get very drowsy after taking the Trazodone).  More research revealed a new medication, dapoxetine, which appeared to be the sure-fire answer.  Unfortunately, Dapoxetine is not available (not approved by the FDA and may never be) to try.  There are other SSRI medications which indicate that they have 'sexual side-effects' but I haven't tried them (Cymbalta, Zoloft, Celexa).

Here's the bottom line:  My situation is so acute that I have frequently reached ejaculation without even penetration or physical stimulation.  I know that my own anxiety over this escalates the situation but I really feel that I need help and not sure where to go.  I've tried physicians here in the states who of course have addressed the situation with the 'standard' methods which don't seem to work for me.

Is there anything else you can suggest for me?  Fortunately, I have a relatively short 'down time' but even the second round usually doesn't last very long.  I think about the times I was able to delay ejaculation and how much more I enjoyed the encounters.  This is also very difficult to discuss with providers so it can be quite embarassing when we first meet.

Any hope for me?  Again, sorry for the long post!

Thank you for your post and your willingness to open up about this issue. It seems you have fine combed not only the threads about PE on this board, but done a massive amount of research in other places as well.

Unfortunately, I don't have much to add to the matter. I am a psychotherapist and not a physician, so all I can offer is what you've already tried - the Masters & Johnson stop-start-squeeze treatment protocol.

I find it interesting that these British physicians prescribed SSRI's AND Yohimbine together. Yes, the stimulating effect of Yohimbine on someone sensitive would pack a wallop. Not to mention the side effects of taking SSRI's if you really do not have primary depressive symptoms. In addition, Trazodone is sometimes recommended for depressed individuals who also suffer from insomnia. I'm curious as to who is prescribing these "cocktails." Taking SSRI's or any psychopharmaceuticals when not indicated for primary symptoms is never a good idea, PE or not.

Since you address a psychological component in your posting, I would like to go along with it, and suggest the following:

Going from provider to provider when one is nervous and tense will definitely contribute to the problem at hand. Maybe you can find an ATF, with whom you can relax in a very deep and soothing manner. A beautiful and emotionally sensitive tantra practitioner might do the trick :-) since tantric sex concerns delaying ejaculation. The tantric environment is also soothing and takes away the focus on "performance," "shoulds" and "musts."

I have this sneaking feeling that if you get with one lady with whom you can have a deeply sexual and mutual connection, the problem may not seem so overwhelming. OK, so you'll ejaculate very quickly at first, then recharge, then go for a second round that will last a little longer. But at least the focus will be on having a languid, sensual time, rather than a driven chase with a good amount of despair in it - although your frustration is certainly valid.

Please try the natural route before you go for the synthetic brain chemicals again. These are powerful substances and their use can have some unintended and troublesome effects.

Thanks for your timely and informative response, LG.  Let me further a few items and respond to a few points you made.

Yes, you are correct.  I have done a vast amount of searching in the hopes of 'curing' this problem.  I respect your opinions and certainly understand that you cannot prescribe medical treatment nor should I seek it via the internet.  While I certainly don't want to gamble with my health I sincerely do want to find some alleviation to this problem - and hence was willing to perhaps try things that were maybe a bit questionable (though at the time I didn't know that).

Now to the psychological aspects.  There is no doubt that a first meeting with a provider is usually the most frustrating.  Even with sites such as TER there is no way to know just how a provider will respond/react to my PE.  What makes it even more difficult for me is that I'm not generally comfortable discussing it ahead of time.  Many times a session with a new provider will start with the offer of a massage (pretty standard and supposed to be relaxing).  In my case, it's terrifying.  A simple massage to start the session can often result in my first ejaculation and in me wondering just what the provider is going to think when I roll over and reveal that little secret.  Will she think I'm strange?  Will she think the session is over?  Just best to avoid that by suggesting the massage come after the first round.

I know your suggestion to find an ATF in which to find some acceptance and re-assurance is a good one.  I've had a couple of these in the past (though not for some time).  They did help a little (though never enough).  I at least felt more comfortable after each visit that at least the she knew me and knew what to expect.  The unfortunate thing was that even with other activities to occupy us the main course was always anticipated (and eagerly enjoyed) but never lasted long enough (at least for me).  The hard part is getting to that point.  How do I let a provider know that I'm really only ready for the main course after I've already had two rounds?  Not many think in these terms so I just accept things as they are and remain frustrated.

I appreciate the advice and understand the concerns over the use of medications (not necessarily intended for this use).  Believe me, I am not one to randomly try different medications without doing some in-depth homework first.  Of course the temptations are there with all the latest advertisements for herbal remedies, long-lost secrets, etc that the internet extolls.  Finding the real solutions is the difficult part.

This has been a lifelong issue for you and your frustration is palpable and very, very understandable.

You ask the following: "How do I let a provider know that I'm really only ready for the main course after I've already had two rounds?  Not many think in these terms so I just accept things as they are and remain frustrated."

My answer is this: if you choose a provider with stellar reviews, someone slightly more mature, say 31-35 and up, someone who allows multiple pops, and someone with a reputation for being SOPHISTICATED and UNDERSTANDING, this should not be a problem. You may have to do some research for your particular geographical area, but I bet you can find someone on TER's vast database. Book with someone who offers multiple hours as well. Skip the massage if you don't like it. As a matter of fact, most "courtesan types" don't do massage at all, unless it's the brain we're talking about. They try to create a relaxing experience by getting to know their clients from an interpersonal perspective. And of course, within that perspective is a frank discussion of sexuality. It doesn't have to get crass. Smart providers are perceptive, non-judgmental and can go with the flow very easily, all with the intent to make you feel comfortable.

PLEASE don't be shy about this. Tell the provider up front that you go through the first couple of rounds with some speed. Because you ejaculate so quickly the first couple of times, this should not be a problem to a provider whatsoever. True professionals are used to all kinds of anatomically varied expressions.

I believe that you have been frustrated so long, that it's created a self-fulfilling prophecy of anxiety as far as making your requests be known. PLEASE DO NOT WORRY. Providers prefer knowing things upfront. That way, they can ensure that you have the best experience possible. And believe me, this is not a hardship. Imagine the opposite - someone with retarded ejaculation. Your particular problem won't make a provider sore, tired or worn out. Quite the opposite. Some women really get off on men who pop and pop. It's rewarding in that it can give a woman a feeling of "achievement."

How charming ;-)! You can do a search by posting on the discussion group for your geographical area. Simply ask, and I bet some helpful posters will give their recommendations. Do not use an alias though, that raises suspicions. Of course, if you have no reviews written, that's difficult. You can also do some "back channeling" in chat rooms, PM's et al. TER is just a treasure trove for finding all sorts of people!

This worked for me:

Masturbate using your hand and some lubricant using very slow steady strokes, then stop as soon as you begin to feel like you are about to come to the point of no return.

Stop completely, even distract yourself if you need to (Look at baseball trading cards, they work.)

Then start over again and repeat for as many times as you can.

Over a period of a few months, you will learn to recognize the signals your body gives you as you approach ejaculation and learn to control them.

It's a form of yoga and meditation.

What's the FDA problem, I think they are worried they will create a recreational drug. They (FDA) don't realize how many people they may help.
