I don't know how many pounds the camera adds but I have been pleasantly surprised a few times.

In my experience it has been the opposite.

Twice, the first time was maybe 7 or 8 years ago.  After we completed the session she counted the money and gave me back half, not sure why.  The second time was 4 years ago, I gave her three and she refunded me one hundred because she considered me a regular.

It’s rare to come across providers who are in better shape than their pics; but I have a few times ...Brazilian providers named Emma and Natasha. I’ve reviewed both. In my experience you encounter this more with the providers who are not from the USA.

Good luck finding an American provider who is not heavier than her pics; the best you can hope for is that they look approximately as advertised...

The last one is debatable, because it had happened when I tipped. So not sure if that qualifies.

I often tip a little on spec by just putting it in the envelope, especially if I don't have the exact change.  I have had several say I gave them too much and try to hand me back the difference.  

3)  Covered on a review I posted years ago.  Planned on a solo date.  Door opens there are two ladies.  Once I was in the room I was confused.  Turned out to be three sum sampling date.  A BOGO.  Had a wonderful time so I paid their going rate for duos.  

1) Years ago I photographed some ladies for the hobby and was surprised the look in the picture was more than the person.  I have known this ever since I picked up a camera (1974), however nudity seems to exaggerate more.  In fact there have been several ladies I have seen that are thinner in real life.  I would name names, but I don't think that would be proper.  

The thinner provider I recall the best was REALLY thin, as in Karen-Carpenter-like thin.  Like you I was concerned if she was ill.  But another provider I knew well knew this thin provider and filled me in.  I didn’t see the thin provider again.  

 
I don’t always know a provider’s true age so it’s possible I’ve met one younger than her stated age, I really don’t know for sure.  More often the advertised age is understated, in one case by over 10 years. But that provider could easily pass for her advertised age.

 
Multiple providers have grandfathered me into pre-increase rates, but I’ve paid their current rates because I thought they were reasonable. A couple of times the provider mentioned it.  And one time a provider gave me a partial refund because she was a little late, but that’s outside your question.

A couple of years ago, there was a lady I was seeing on a pretty regular basis for a few months. She was accustomed to me tipping her well in addition to the donation. Over time, she also trusted me to the point that I would pay her after the festivities, not before.
As things sometimes happen, we guys stop hobbying for a while, so when winter approached, I stopped seeing her.

I saw her again the next spring, and on that first new session, we picked up where we had left off. At the end of it, I had genuinely forgotten how much her rate was. When I asked her, she sort of laughed as if I were joking and said her rate was the same as always.  Hell, I still couldn't remember, so when I went to shower, I prepared the envelope, which I gave to her when I returned to the bedroom.  

"This is way too much," she said in surprise. Damn, I said to myself, but I shrugged it off. "It's fine, I want you to have it. I didn't see you all winter, so consider it your Xmas gift." It wasn't some extravagant amount, and it was in my budget (my hobbying budget), so after that when I saw her again, I subtracted what I thought was the over-the-top extra amount, committed it to writing in my burner phone next to her entry, and it became the new baseline for how much I'd give her every time I saw her.  I never did tell her that I had forgotten, and she probably thought of me as a generous old man.

I never have done very well with handling my diminishing memory as I get older. I'm often too embarrassed to admit to anyone that it's failing, and I do a lot of things to cover up for it.

Three or four thinner looking than their photos.  I thought that was a plus.  Others appear a bit more ahhhh "healthy"

Younger than stated age? Yes, several times - in some parts of Europe.  Once in Greece years and years ago. The pair I met turned out to be a Mother and daughter (or more likely older woman and younger gal). I figured the daughter 15/16.  

Once. It was the third time with the same provider. Same as some others above; she told me she considered me a regular friend now and returned 50.  Saw her fairly regularly for three years.

My local ATF had told me to start paying her less when my income decreased. Prior to that, as I was making better money, I would pay her about 130-150% of her fee. I did so because she normally would be with clients for 30 minutes or less, but we always hung out for at least 2 and usually 3 hours.  Granted, we only spent 20-30 minutes taking care of business, but I also knew that she could make more by not seeing me most of the time.  When she knew my income was about to drop, she encouraged me to just pay her the normal fee even though we would still hang out for hours.  It’s just one of the many reasons I’ve been seeing her regularly for over a decade.

I did have a pro cut all future meets by 16% after our first.

I've seen two ladies that were thinner than their pictures. The first was Marianna The Muse in Manhattan. She was noticeably thinner and tighter. Not sick looking but rather more like a gym rat and runner.

I saw a lady this morning, Alexis Summer, who looks a little thinner than her pics (at least I think so) but she does have a great spinner body.
