Should photos be retaken after 3 years? 5 Years? Or more importantly....
After how many pounds??? 
If a provider (let's use 5'5" tall woman) gains 25 pounds are her pictures no longer truly "accurate"? after 35 pounds?? 50 pounds???
And, are there other characteristics like hair length and color? Significant changes in tattoos? New teeth? ...that warrant a photo shoot?
So just how much change and what kind of change demands new photos?

Surgery, kids, tattoos, boob job, weight loss or gain 10 lbs or more, hair color, etc. If your body looks the same as it did 5 years ago and you blur your face, no one is going to know one way or the other if your face looks older. It's good to keep a main set of pro photos, and then add new ones in your gallery every so often. I know many gals who have used the same photos for 10 years on their sites, but do the ads and twitter with new photos.
has but on significant weight, say 50-60 pounds? Do you think it's reasonable to decline going through with the session if that is discovered at the door? If so, what specifically do you recommend the gent say, if trying to remain professional and gracious about the issue?
Are you kidding? Even 10 pounds is just cause to decline, if you are not attracted to her. It is better to decline vs. going through with it and having to write the review. You saw what happened the last time a guy dared to be honest, yet polite with his review? Just walk.

Then the pictures should be changed. I have seen ladies with 3 year old pics that still look exactly the same, so it's not an age thing. It's an accuracy thing. Does she look older than the pics? Gained weight? Lost weight? Different hair length/color? Implants? Breast reduction? Nose job? face lift? (of course those last items only pertain to those that show their face) If she is has a significant change in appearance from the pictures, then I want to see new pictures, If she looks exactly the same as the pictures then I could care less when the pics were taken.

I've can't recall a review where the guy was bothered by hair style or color. But if a lady is say 40 or 50 pound heavier it is a real problem for some guys. Similarly, the pejorative comments I've seen about age seem to be when the lady looks 10+ older.
Seems like other issues are taken in stride a bit more

Of course age and weight are the two most significant issues, and that was the first two I mentioned in my reply too. But your examples are exteme! 40 or 50 pounds? 10 years? Some ladies will look much different with ten added pounds and some won't. Some will look way different with two added years. This is why I said new pics are needed when she looks different, not based on time since the last pics. If a lady looks the same as her five year old pictures then she doesn't need to change the pictures. On the other hand if she took pics yesterday with jet black hair half way down her back, and today she cut her hair really short and died it platinum blonde, then she needs new pics!
And I HAVE seen reviews where people complained about hair styles and colors. I realize some ladies change that kind of stuff often, but if they do, they should have a variety of pictures showing the different colors and styles they may show up in, or at least mention in their sites, that they change often. There should be no surprises at the door.

It would make no sense for me (and be a waste of money) to retake photos all of the time.
I don't show my tattoos, but if a gal did... should retake pics.
I've been the same for the last 16 years (not kidding), so I update mine every few years, However I will be redoing mine as I have a tan (currently) and it changes my appearance. Plus I am going to be going to the gym and harden up lol.
Technically I should update pictures every few months (as my hair naturally changes color, from dark brown to golden brown). I was going to update when I got my hair straightened (it would be a big difference as my hair is ultra curly) but it never stays that way but for a few days (even professionally).
Anytime it is a big difference, the gals should update.
Or did you brainstorm with Captain Obvious and Admiral Ambiguous before unleashing that flash of brilliance on us all.

I agree. I'm sure it's suspect at times for men to see old images, but the reality is some women don't change. I could work off images from a shoot I did in 2004. Still 119lbs, face intact
New pics every 10 pounds or 10 years.
I personally re-take new pics every 3-4 months(sometimes 5 months when time escapes me), even though my appearance has not drastically changed at all since I started. My hair length has went from slightly below the shoulders, to waist length currently... but other than that, not much. Personally I feel that even if your appearance has not changed drastically, or even in small ways that may cause gents to complain about inaccurate pics, new pics keep everything current, & nothing can ever be said about the lady having old, outdated, or inaccurate pics.
If a lady gains more than 10-15 lbs(depends on height & what the weight was before the new wt gain), ages visibly(don't keep posting pics from when you were 21 and you're now 31), gets tattoos or piercings, changes their hair color(say, from blond to black)... anything that alters the physical appearance enough that a gent could say the current pics weren't accurate.
