Well, for starters, all the usual suspects for mouth to poop contact are available. Stuff like Hep-A, choloera, E-coli etc. These are generally resistant to mouthwash, so while that'll make your breath fresher, it won't get rid of the stuff that'll hurt you.

Generally, if the person is healthy and hasn't eaten contaminated food, you're looking at E-Coli being your biggest issue. That's basically playing Russian roulette with your digestive system, because usually your immune system mops up E-Coli before it causes problems, but sometimes it doesn't. This gets more likely if you're already sick (ie have or are getting over a cold), elderly, or are generally immunocompromised.  

This equates to significantly more risk than DATY or BBBJ because of the additional bacteria and potential for the anus to have tears (and therefore blood exposure) without the recipient knowing.

All of this risk and some of the eww factor is easily mitigated by a dental dam and some lube. The sensation is more or less the same, and you really stay much safer.
