Maybe it's just me and my computer, but the provider VIP search form is often wacky when I try to use it.  The dialogue boxes aren't lined up, are double & triple images, they don't work, etc. This is only when I log in as a VIP member; the simpler search form works fine when I'm not logged in.  Anyone else having this problem?
