Go to the provider's profile and click on "report a problem." Be clear and concise when describing the changes that should be made based on your experience. They already will know which provider it is. You might even get two days added to your VIP membership, if your report is approved (and it's the first one submitted).

I asked this question of the board and got an inadequate answer. Here's the deal. Recently, a reviewer posted a review of a new asp. He had sex and no more. So, if you read the review it says sex (yes) and all the rest of the services are listed as "don't know". It appears that that information is then set in concrete. Then next guy comes along and has a lot more fun, doing a lot more stuff, But, the asps Activites section remains the same. Should be a way to change this. Same goes for a reviewer who inputs information on physical attributes, but gets it wrong. Then, the inaccurate information remains.
When I asked the board about this, the answer was that I could go into my reviews and change ones I'd written, but that doesn't asnwer the problem.
