I have had goood luck of late with the Visa prepaid debit cards.  You have to activate them and give an email and address to get them to work, but I just use a throw away email and make up an address - it works (it didn't used to, but now it does).  I buy from a reputable store (Walmart, Safeway, - I had bad experience with cards from smaller conveinience stores)
