I have good personal hygiene though I am overweight and have always been susceptible (even pre-hobby) to heat rashes in the groin area. Lately my "sack" seems to get reddish after a vigorous workout, even if I am using prescribed cream and even if the surrounding area is not red. STDs have been ruled out by a very recent blood test.
Thoughts?
But what exactly do you mean? (A) Go on hiatus. (Tried that, cam back and so did the problem). (B) Use copious amounts of a-f powder. (C) Something else?

What is this "prescribed cream" that you're using that's not working? A cream that I know of that's good for groin rashes is Derma Cloud aka "Fanny Cream". It's not cheap but it's otc.

Moisture is bad. I work out at the gym almost everyday and I perspire a lot. It takes me a long time to cool down. So I go to the gym, work out, drive home 15 mintues and then shower. I make SURE to be dry in the groin area so that the rash doesn't pop up. Bacteria grows in moisture and you have that tendency -so take the precautions, as I do.
Even baby powder works! That's why you put it on baby's..so they don't get diaper rash. That's a form of what you have! ![]()
If I'm not careful I get it in the area under my arms. So I make sure to be dry before I dress. i use a poweder for men - Mennon - works great.
But the operative word is "dry."
...a vigorous workout. The cream I'm talking about blocks moisture (for a limited time period, of course), so if it were used immediately preceeding his workout it may prove helpful. After he showered, then yes it would make sense to dry the area... perhaps with a powder. However, if you want to prevent or treat diaper rash guess what works the best? You got it... DERMA CLOUD aka "FANNY CREAM". That's why I recommended it. I'm a stay-home dad with two small sons, so I know what works. This stuff also works great when you go to the beach and want to prevent groin rash caused by the salt water and moisture in general. Of course the number one thing all guys should do at the beach is to never wear those swim suits with the built-in netting. Avoid jock straps, too. That traps in the moisture. Wear the newer, fast-drying materials; those suits can dry off remarkably fast.

Along same line, I tried shaving pre-visit per some of the past posts about the ladies preferring not to be flossed-
resulted in beet red scrotum and zits in the shaved areas!
Any suggestions?
Shaving did cause red bumps but practice helps.

is to shave down the scrotum and groin completely (have never gotten zits or bumps there), and just trim down the rest nice and short. Thereby avoiding razor bumps, and also irritating stubble!
Being an extremely busty lady gives me some familiarity on this subject. The one thing that works well for me is Gold Bond Medicated Powder. Make sure the area is completely dry, then apply the talc liberally.

I have had a few gents with just such an issue and here is what I learned over the years:
Antibacterial wash or glycerin soap with no scenting or coloring to cleanse. Sometimes 2x daily.
A good astringent, similar to like you would treat acne. Witch hazel works well when applied with cotton pads and will draw out skins impurities.
Some powder to keep things dry. Regular or medicated gold bond should be fine. Moisture is the devil in dark places.
May also want to find out if it is a sebaceous gland infection which can be sometimes be found in the armpit areas and groin regions. Common in adults, no cure but easily managed with time and the right product and sometimes a heavy long term dose of antibiotic![]()
