I prefer that the first, the other second (since the second time takes longer). It's pretty hard to disrespect us older women! lol

Some providers arent bothered by CIM. Some providers seem to think it is the absolute sign of disrespect.

Is CIM disrespectful, even when a provider is willing to allow it?

I don't see anything disrespectful about CIM at all. For many reasons I won't go into, it is something I do not do, but my reasons are not because I feel it is disrespectful. To me, CIM is very intimate. It is like taking a part of a man into me. I see it as a very beautiful thing, one of the most loving acts that a woman can do for a man. I'm sure that not all women would agree with me of course, but those are my thoughts on it.

but many women do like it and it is a very powerful expression of the feelings the client and the provider share for each other.

It's not just about debasing a person, although even if it were, there's a whole argument that even that is a legitimate practice as well.
That would get us into the whole BD/SM question which would take several long threads to work itself out.

I realize that I am not a provider, but I just couldn't help myself.  Sorry.

CIM is not necessarily disrespectful- it's all a matter of context. You say "willing to allow it," but some of us actually enjoy it. There are some situations where a lady might actually "take" it from a man, and in that case it can be a very dominant act on her part. I do agree with above, though, that to do it without asking is very very disrespectful.
Ultimately, I think it's a matter of the dynamic between two people, the same way that rough activity is uncomfortable or intolerable for some but the height of ecstasy for others.

Its not disrespectful...Its rude if the guy does it..without asking of he can do it..its rude if the guy does it after the lady said no, or she warned him to let her know when the rockets would flare...

Its  a preference..if a lady likes it and tells you she wants it, well then by all means give it to her!  :)

... but, please ask anyway. It's the polite thing to do.

i for one, don't allow CIM. It's not disrespectfull if it's on her Menu. If you try to talk, force, purposely knowing it's not on the menu, etc., that's disrespectful and you won't just get a bitchin either...
