I am racking my brain trying to think of a single "good" reason why former BF would do this.  Sounds like it is to hurt, make trouble or possibly shake down someone.
I would thank him for the information, give minimal information out and show him to the door.
I would bring the situation up with the provider, the STD "report" would be a piece of it, but the real problem is the former S.O. still acts like he has some rights or privilages.
IMHO.

Got a situation with a provider and her former BF.  The provider currently has a "customer" who she is seeing socially for nearly a year and their relationship is serious.

The former BF discovered this new relationship and went to the office of the "customer" to meet him.  He introduced himself as her former BF and told the "customer" that the provider may have a common STD.  Okay, at this point would you (1) ask the former BF to leave and approach the STD problem with the provider; or (2)give the former BF your time for him to say what he wants to say.

I would thank him and approach the STD poblem with the provider. How much time I give him depends on his attitude, however I would not invite him to talk over tea.

A client started a real BF/GF relationship with a provider, has been seeing her for a year. (Usually in this situation this means unprotected sex.) And both of them haven't been tested for STDs?

Are they both insane?

If they have both been tested and tested regularly, the entire issue becomes moot. If as BF/GF they have protected sex, then it also may be moot.

Steps to be taken at this point.
1. TRO on the ex-BF. Tell the office receptionist not to allow him in the building or office.
2. Have absolutely no contact with the ex BF and neither should the GF. (Don't give him any ammunition, this is what he really wants, to be in her life even if it is BAD drama.)
3. If you haven't been tested since becoming BF/GF, then you should go down to the clinic, TOGETHER, and get tested.

This is your life we are talking about.

Hes probally lying..and trying to get back at her..most likely..but tell him to leave, and then tell her what he said...and see what she says.
