The Code of Hammurabi omits the number 13 in its numbered list.  The code dates from about 1686 BC.

Yes, it was first mentioned in reference to the Last Brunch, when Simon-Peter ordered 13 pancakes but ran out of syrup after eating only 12. It was considered a bad omen.
