I almost had extras only once and chose not to, very peculiar.  I get real professional therapeutic massages often, naked, no extras. Once, I was getting a message as prescribed by a chiropractor and given by a professional certified female massage therapist in his office suite.  She began to massage me very very hard which I usually like, but she began to cause more than usual pain.  I finally said something quietly and she said she should, perhaps give me a "safe" word.  This is, of course, BDSM procedure.  She became more and more suggestive in her language and expressions. I had just started a job with a new company selling advertising. I'm a Navy vet.Spent some time in Hong Kong.The 1st thing i would do after getting off the ship for liberty(R&R)....i would head to a massage parlor!
The place wasn't upscale,but not a dump either.

A certified massage therapist giving "extras" would be like a physician, dentist, psychologist. podiatrist, physical therapist, etc doing something inappropriate.  There are certified sex therapists, of course, and that is different.  I am trying to imagine a provider giving medical prostate or skin examinations as a "finish." But it is fun to talk about and the question was interesting as were the answers.
