Similar issue for me, but a very experienced FBSM lady showed me how she uses her hands (well lubed) in combination with her mouth for the additional pressure to the shaft.  She was so good at it that I couldn't tell what was mouth and what was hands - and didn't care!  First time I saw her, I nearly put her eye out when I popped.
