Am not sure how this works If have a dinner date Is that when the time starts or after we get home

Usually dinner dates would be starting with dinner, then thru the evening. Check out the times on the websites, they usually specify dinner and how long you would be with her.
Me...Im a 4 hour with dinner. Cocktails would be 3 But thats just me....some could be longer, some could be shorter. YMMV.

The clock starts as soon as you meet. This is where the expression "paying me for my time only" comes in...
Now, if you get to be a regular it is quite possible that she will go to dinner with you off the clock but that is never something that you should assume at the outset with a new provider.

For me, a dinner date runs about 4 hours...give or take.  Lunch or cocktails about 3.  I usually prefer to only schedule one multiple-hour date per day so I do not have to rush or feel rushed.  At least schedule far enough apart to rest and relax a bit.    
To answer your question of when the time starts?  As soon as we meet.  Dinner at 7?  Time starts at 7.  Simple.  It's  an appointment.  But as they say, YMMV.  Some ladies may choose to spend some extra time with their friends.  That's a personal decision.
