I just realized that if you try to review a provider and she does not have her name in the add you  can not put in a review.  Do I have this correct?

If she has not been reviewed previously, then your review will not be approved without a name in the ad. If she already has a TER profile and reviews, then your review may be approved based on other matching information in the ad such as a phone number. Either way, I would submit the review and see what happens.
