I was with a skinny provider here in Arlington last month and in the middle of some fantastic BBBJ I busted out laughing and upset the provider. MP's approach works, but it also sounds like you've been seeing high-volume, lower priced providers. You might try gals who charge at least $300/hr. They tend not to need to be clock-watchers (even good ones where you're not aware of it). If they like you, they'll let you go somewhat over, or at least not make you feel like you've been hustled out the door.
