Anyone else have a bad experience with a provider before? How did/do you handle it?

Long story short:
This was the second meeting with a certain provider.  I requested 3 hours because the first meeting ended before I felt I was done. I was more familiar with her this time so after a quick bj we got down to it pretty quick, me on top plugging away.  
She was really into it at first and about 30 min later I came, pulled out and changed condoms. She was a bit surprised that I was still ready to go but she was still into it. I got back on top and started drilling away for the next 45 min. She was panting heavily and we were both sweating by now.

Is this a case when I should write a bad review?
Is there some limit to the amount of penetration allowed per secession?
Should I ask about this limit in the future or is this just b.s. and she was just faking it?

I really hope you learn your mistakes here for future providers sakes.

1. If she pushes you away slightly when you are penetrating deep, it means "NOT SO DEEP." Show some respect dude and learn to read body language.
