Sometimes I don't feel like doing any physical work. This stems from the fact that my main reason seeing a 'courtesan' is for me to relax at the end of the day. By then I've already visited the gym and ran a few miles, so I sometimes don't want the additional physical work out involved during sex.

Many providers in my area offer this off the books, meaning don't advertise it. Even some higher-end (just based on rate) providers, including PSes, do this if they're not in demand. It's just the reality of the business.
OTOH, there is something to maintaining the value of your time as JLS said. A compromise is to just do it so well that you don't get complaints. There are some guys with erectile issues who can only come from oral or a HJ, or a mixture of both. Those sessions can be quite lengthy and extremely physically/emotionally draining.
