But then every once in a while I see a provider who is just, "ready". No prep, no nothing, and it's perfectly clean or at least no sign of anything untoward. You eat you make shit lol (we/providers do find time to eat lol) .... very likely someone whose just "ready" has prepared herself in anticipation (whether you might want it or not...I know some ladies like that).

Preparation: some may not need as much notice (depend on diet and a bunch of other things) and some do.  But yeah there's always some kind of preparation unless you want to ride the Hershey highway.
