The forms are for the lady's safety and peace of mind. If you stick to enquiring with well-reviewed ladies, then you should never have a single worry. But if you truly are a CL dude, you may have cause for pause.
Fill out the form for well reviewed providers but try to avoid giving too much info to the CL types. if you are able to connect with a reliable provider she may be able to give you a reference for the future.
Not sure if you are alluding to the aspect of submitting the form directly from her site or if you are worried about sharing screening info at all regardless of exchange method.
In terms of the former, I do not submit her form directly from her web page but instead choose to send the requested info to her email addy from my email addy. There are aspects of this type of exchange that make me feel more protected.
In terms of the latter, it depends upon the requested info. Most hobbyists will have their own limitations as to what they are willing to disclose during screening. Some guys will be okay with providing certain info and other guys will balk at it. In this light, I'm okay with some info requests and not okay with others. However the key to me in getting past these fears is to deal with providers with stellar reputations and consistent feedback/reviews. A well-reviewed provider with a decent experience history, is likely a totally professional lady who will protect your confidentiality.

PLEASE email the lady and ask her if she received your request? She may respond that she has and is busy, she has yet you didn't provide all of the information requested, OR you may alert her to a technical problem that is happening on her website!
I just yanked my webform for now and reverted to email and phone because for no known reason, now and then, a web request seems to go into a black hole...
I'm happy to give the info on the phone. I'm happy to send an email that gets a call back. I don't put that kind of info in an email nor on a web form. I realize that I miss out on a lot of great ladies because of it, but that is my choice.
Basically the form is for the convenience of the provider or agency, which is fine. Their convenience is not worth my piece of mind. On the phone I can get the sense of the lady or agnecy in question. Chat a bit. See if she seems to have her act together. Once I'm comfortable with her then I'm happy to tell her the info she needs to feel comfortable.
Call me paranoid but who knows they may be out to get me.
it is folish to submit via a web form or email in my view, you never know who/where it may be passed on to, preserved, traced. I too prefer to talk by phone and provide info verbally. Its just much much safer. Most people (myself included) really don't know how to completely wipe stuff off a computer (and there are many tech articles that tell you merely deleting it does not do it beyond recovery). The provider/agency's computer may be seized, stolen, sold, or discarded and then reclaimed - you never know who might decide to recover/restore info that the owner thought was deleted but was in fact not deleted.
the form to be filled out... but rather call me with the info, for this and other reasons not mentioned, and its working out well for me... i used to always require the form filled out by email, but they were always filled out incomplete,anyway. i really enjoy the phone contact, and not the email, and phone, as i dont have time for all that. xoxox allisa love
I wouldn't mind providing all that information, but I rarely seem to get a reply from those forms. I always seem to have to follow up with an e-mail. So when I don't get a reply, I wonder just where all that information wound up, and it does make me nervous. So now I avoid those forms and provide that information in an e-mail.
