A gift bag or gift box is a simple, discreet way to handle it if we're meeting in public. Cards are sweet as well, of course, although they can get a bit bulky with longer dates. But you get bonus points if you bring one that makes me laugh.
I also had a gentleman once hand me a newspaper and say he thought I'd be interested in the article on page 3. I was very interested! Haha.

on more than one occasion I have included the donation inside a large box of Dark Godiva chocolates, and of course the chocolates inside were still sealed in a gift bag. It was for a lady that I have seen many times since.
Also, I have used Gift bags when I could not find the right greeting card.
I have never used a plain white envelop or just the cash. IMO I think handing/flopping cash is crude and impersonal.
I have received a donation in a box of chocolates before. The brand wasn't Godiva but it was equally (if not more) delicious!
If I know the lady well, I get her a card from a stationary store that is relevant to her. I have found that any attempt on my part to personalize the experience is almost always met with a positive response.
I roll some benjamins around my shaft and yell out "C'mere bitch, I've got something for you". Not a one has complained about the lack of an envelope. One did mention something about the taste of the glue.
I used a birthday card, I've also used a christmas card with a small gift.
Recently I traveled to a city for a nieces' bat mitzvah, and while at the synagogue, I picked up some paperwork about the place including a fancy envelope used for giving donations.
Well, the next AM I was off to a session with a gal I wanted to see in that city for a long time; and I planned a trip to an ATM, but needed an envelope. I just assumed the hotel would have an envelope in the desk draw as they always used to have, but not this place.
I think you all see where this headed now.
She was pretty bemused by it, I must say.
If so, perhaps she thought it was a belated Bat Mitzvah gift. Which reminds me of the old line:

PS: As to the envelope question, I always read a provider's web site before visiting her. If she requests the gift be in a white envelope, I do it. I mean, what's the point in violating her requests re safety (even if we all know it's a fig leaf that means nothing to LE)? What's the point? Why would I want her to be put off, right at the beginning of a session?
An envelope stuffed with bills is a pretty powerful source of conversion.

Most of the time I use a greeting card. Occasionally just a plain envelope. Once or twice the bank envelope I received the cash in. If it's a birthday or other occasion I give that separately sometimes with a monetary gift in it.

...tend to use a greeting card of some sort for me..many of them having to do with something personal to me..like bicycles..wine, foodie stuff.. nice touch and always makes me remember the guy!

I find that paper envelopes and ziplock bags aren't strong enough to hold all the rolled coin or loose change that I prefer to pay with. I use a zippered key-lockable canvas bank deposit bag or the newer polymer (nylon) ones. Some of the ladies consider them to be very chic and haute couture replacements for a traditional purse or pocketbook. They come in a variety of colors to match any outfit and "natural" unbleached canvas is so politically correct. Although no one has told me flat out, I'm sure they appreciate the muscle-toning workout they get carrying ~10 pounds of coins to the bank for deposit or conversion to paper money.
Note: Stay away from the "leatherette" ones. Not as strong as nylon or canvas and very declass('e).
I have often found that a little Teddy Bear (with a pocket that's just right for tucking the donation into) is a good choice and has often been received so well by the host that I feel pretty certain that I got extra special treatment in return.

I have the correct donation ready in my key pocket with a simple paper clip. If a new meeting I place it per instructions. On repeat meetings, I normally empty my pocket junk on her dresser... including the donation... when I redress, I refill my pockets leaving the donation behind. I do not watch the donation per say but often in the back & forth of dressing & bathroom use, it will silently disappear. Old school but it adds to the ILLUSION.
Once I had a girl I'd not seen in a while swoop in to claim her donation, rather than waiting for me to finish picking up my stuff & stepping away. Too bad... she proceeded to light up with her roommate in the living room while I was in the bathroom... more interested in smoking than to give me a hug & a kiss before I left. Nope, scratch that one.
I almost always forget to bring an envelope with me, so I usually end up using a folded up piece of paper. Sometimes I even use lined paper from a notebook binder haha.
