I've seen the upsell and I've seen the "one price" providers. I've had amazing sessions for $100 and lousy sessions for $200, which is why I'm a rabid reader of the reviews here.

New York City will cost a lot more than say St. Louis.  Las Vegas and LA are also on the high end but Miami, Chicago, and some other large cities have good providers for a reasonable fee.  No substitute for doing your homework.
