For any of us who are experienced in hetero BGB, I'm wondering if there is a typical approach, and if so what is typical?  

Does the provider bring the other guy?  If so is it a male provider or her boyfriend/fuckbuddy?   Or do they look to the client to bring a friend? and have to screen us both?

Probably no single set approach, but is there a majority view???

Option 1: Seek a provider who advertises more than one guy in a session (assuming you have a buddy who hobbies as well) she verifies the both of you and the date is set.  Some ladies advertise their willingness to go to swinger clubs as well.

Option 2: Depending where you are located, many Top Tier providers have a male provider with whom they work with, Vegas comes to mind.

Option 3: Seek out a hetero BG couple that advertises the services that you are seeking.  A fairly well know PSE provider from DC offers this option.
