It hasn't happened lately, but my knee would sometimes pop out during prolonged encounters.  Very painful in the moment, but easy to fix, and never a lasting injury.  I actually can't recall any injuries that lasted beyond a few minutes.

Broke plenty of furniture and stuff, though.  Lamps, clocks, dishware, chair, bed, and so many spills on rugs and carpets.  Sex has a blast radius.

My experience wasn’t with a provider, but my worst sex injury resulted in a dislocated hip and trip to the hospital, during an ice storm on Christmas night in 1997.

I'd actually say Doggie or some variation thereof -  however it does still have one danger point for injury.  Because you can get deeper, and since we're talking aggressive, it is possible to bruise the cervix, which I have done.  In the moment it feels awesome, the next day there is a distinct soreness and requires gentle sex for the next few days until it heals up.
