I carry it with me, incall or out. Invite her in to the bathroom and pour a splash for her and yourself at the start of the session.

my immediate concern would be what else about her is not clean and fresh?   Of course, if she is a smoker that could be the issue.  

and take some yourself.   Another item that works well is a stick of cinnamon gum such as Wrigley's Big Red.

Yes, +1 altoids. I love altoids. The peppermint ones are great and add a fun little boost to oral sex. You might offer her one even if her breath is already pleasant. And you could grab one for yourself before DATY (if your neck can handle it).

I always have it with me. Any session.for me or my partner. Nobody will say no to mouthwash if you ask politely.

At the end of the session, shoot her a text  
"yo girl, you got stank breath. brush your damn tongue, and drink some mint tea"

I would politely suggest that you and her both use the restroom and use mouth wash. Or go together and do like a cheers to mouth wash sorta thing. That is always cute and fun and doesn't ruin the mood.

I think this a very easy to accomplish without telling someone. As mentioned before on the mouth wash. I always carry breathe mints on me all the time on a date. This is for both me and her, I simply ask if she would like a breathe mint as I take one as well. In all cases it has not ever been turned down.

You can keep it short and sweet like I love LFK/DFK and I will always show up with fresh breath. Proper Hygiene is VERY important to me!"

And at the end

I usually have mints readily available and consume them occasionally, during the session. Every time I pop one in, I offer some to her and she accepts 99% of the time.

bad breath, they just mask it for a VERY short period.  Sort of like some middle eastern cultures where they think splashing on a little scented oil takes the place of a bath.  

That's more of a white american cultural thing--  Spray on some cologne or Axe body spray, choking everyone around them in the smell of musky wet dog and ass crack.

Overseas much.  Anyone that was lived or worked in the middle east or the Indian subcontinent knows what I'm talking about.

scrape your tongue than to brush it.  Scraping removes all kinds of nastiness that can accumulate in the nooks and crannies of the tongue's surface.  Personally, I like to use a very sharp kitchen knife.  I start at the back of the tongue, scraping towards the tip.  If the knife is sharp enough, you will also remove the top layer of skin, revealing a more supple layer underneath.  Once you are convinced its clean and free of debris, put it in a foil lined pan and slow cook it at 240 degrees for two to three hours.  The thicker that you left the top layer, the longer it takes to cook through, so make sure you scrape it enough.  If you're serving more than three people, you can prepare and cook two tongues in the same pan.  Just add 15% more cooking time.  Scraping is a lot better than brushing your tongue.  Brushing can leave unpleasant-tasting bacteria.
