Clients who were "really" bad were - info was shared with other agencies.
Clients were screened by good agencies.
Records were kept.
LE was alive and well and 'active'. (no less - no more then currently)
Agencies would describe the physical description of the ladies they had available and the client would choose who to see based on that, then speak to the lady on the phone.
Code words were NOT used if you wanted to avoid handcuffs. The farthest most would go is to say "full service".
Appointments were outcall.
Contrary to popular belief GFE services were not unheard of.
Independents
usually advertised in local papers or papers like LA Xpress
most ran things similar to as listed above because most started out at an agency.
Some did incall.
BUT, when your independent you can run things how you want, so some independents did things differently

My life back in the middle to late 80's as an escort was in many ways easier than it is now.
I worked for an agency in Knoxville, Tennessee that was ran by a very nice woman for about 3 years until she retired and closed it. She usually charged 200 to 250 for an hour which was of course out call. I always got half of what she charged and all of my tips. Her agency was advertised in the phone book.
For most of those years I carried a beeper, so I could do what I wanted to, until I got beeped by my agency letting me know I had a call to go to. I would go to a pay phone and call in, get the information and go to the appointment. I would call me agency when I arrived as a safety measure letting them know all was alright and also when I was leaving. If anything was wrong or I was in danger we always had a code word to say in the phone so that my agency could try to get help to me.
After my boss closed her agency, I occasionally toured Myrtle Beach and Nashville, plus still worked in Knoxville, for 2 more years, then I quit working as an escort until I started up again about 5 years ago.
In all those years of working back then without all the safety measures we now have with the internet, I never ran into LE, got hurt or had anything bad really happen to me, except for one incident. In that incident, I had walked into an out call and the man grabbed my purse and ran out. That was it!
I also was providing GFE before I even knew about the term "GFE".
Providing back in the old days in my opinion wasn't that bad and actually was alot less trouble than it is these days!

Since I can't speak for any provider, I'm just gonna answer the questions that relate to the client point of view.
As far as types of ladies...we looked at ads and we had to either believe photos, or we had to rely on the woman to describe herself as being accurate.
The two-call system always was in place. It just means call once to get an appointment and an approximate address (usually an intersection), and then another call to verify you are at said approximate address and then getting the real address.
There were no "meet and greets" in the old days.
People still asked details, but they were more sly about it. You didn't ask about whether a girl did a blowjob without a condom and whether she swallowed. You just read between the lines when she said it was a "good time" or "full service." Rates were still quoted as rates ("My rate is 300.")
It's really not that different than now. It was just more primitive.
