How do you guys ask for a reference from a provider?  I mean, what information do I give, without giving too much?

should be someone you've seen fairly recently and therefore, all that's needed is your name.

Most providers are happy to do so because they know that references are what keep everyone safe and make the whole business possible.

Just ask, the worse that can happen is she'll say no.

It should go without saying also that you never give out a provider's name as a reference without first checking with her.

You wouldn't believe how many Mike's and Dave's there are!

Something like, "Mike from Albany who saw her at the SeaSide Hilton in July" or "who brought the mix dance music CD to listen to during our time" or "who has the dragon tattoo'd on his calf"...
Please when asking/providing references, supply just a bit more information to jog the ladies memory.  It is frustrating going back and forth asking for more information and none of us wants to respond with, "John who?!"
