Under the options, please include one that says "Text".  A lot of providers these days are communicating via text message and even requesting in their ads that we text them instead of emailing or calling.  

Right now the options are Voicemail, She Picks Up, Page, and Other.  The term "Page" is also perhaps an anachronism and could be removed.

At least half of the ladies I have seen work this way.  
The Booker or PO is not the boss, usually works for several ladies.

Come to think of it, Agency would be a good phone type to add also.

"Booker/PO answers" & "Booker/PO wants text",
"Agency voice" & "Agency uses text",
"Voicemail",  "Prefers email",  
"Other".

Page is almost certainly obsolete and extinct.
Some ladies insist on email for first contact, and Type of Phone Service might usefully indicate that
