After some research I have come to realize that BBFS is more common then one would think. So the questions is. Should TER add BBFS to the "Actual Services Provided" for times you review a provider? Or is there a good reason not to?

There is no way to ever know if IRL she is involved with someone and not using protection.  It's a PnP world and regardless of the situation there are known risks.  This being one of them.
