I've posted a few reviews and hopefully they've been of some use.  The "fact sheet" for each provider seems like the one submitted for the first reviewer!  It doesn't change when I submit a review.  Nevertheless, I'm asked to submit all these same details.  It's not a crazy idea, it just doesn't serve any purpose if only 4 things (attitude, atmosphere, appearance and performance) are listed with my review.  Why not eliminate all or most of these for review submissions?  If you need an active website then ask for that too, but there's no need for all the rest if you aren't going to use them.  
It would be best, IMHO, to ask for all the entries then use them all!  From your own FAQ, "A lot can change with a provider even in this relatively short period [3 months]."  Do you guys really want a "new provider" begun if, for example, a provider's website changes?
