How about adding "Fluency in English" (or similar) to the form,
with choices of "fluent", "acceptable", and "poor"?

Yes, that may seem kind of silly, but I just had an experience
made much less enjoyable because of language problems.

I tried out an Asian massage place (can't review it here
because there's no URL for it) for the very first time.
Although the place was very clean and the masseuse was
friendly, I couldn't carry on a conversation with her because
of how poorly she spoke English. It absolutely ruined the
session and I don't want to let that happen again.

Right now the possibility of language problems automatically
causes me to avoid any provider with an Asian type of name.
