There is probably "no blame" from a crewman. Studs do break - last year we had two crashes caused by broken studs. The wild one was Tim Wilkerson's son, Daniel - in Tim's old funny car. At first it was thought that the lugs were loose but tests showed that a couple snapped.
There is tremendous torque on the studs / wheels when the car goes into tire shake. It's rare this happens but it's racing. Parts break and fail.
It is possible they weren't on tight but that would be a very rare event and usually you can tell watching a car doing the burnout if they are on loose. I've had a $2000 axle that was "unbreakable"....guess what...break. And it happened at 130 mph. That was very exciting....like bad exciting.
NHRA will look at the studs and probably get the manufacturers to run tests.
