i am looking for advise on what is the best way to ask a provider to give a reference??

i have only indulged in the hobby twice and both were a couple of years ago...one provider is not available to contact anymore,  the other one is still active but it has been so long that i'm not sure she would remember me.

whats the verbage i should use??

Even if you could get references from those ladies (do you have anything special which would help them remember you?!?) it is unlikely that references from so long ago would be accepted.  People change over time, and we like to know how you've behaved yourself recently.

Check with the lady to see if she allows any alternative form of screening, explaining your situation.
