So I will assume that an incall is the better way to go where the provider can allow smoking at her place. Any suggestions?  I am staying downtown SF. Is it just me with this fetish??

Advanced TER search for a provider that "smokes during session."  If you were in LA/OC I would recommend porn star Jade Hsu, but don't know of any currently in in the BA.
