Agencies / Providers asking for Hobbyists last name

During a recent screening application.  I was recently asked, in addition to my p411 & TER info, that I provide my full name (first & last).  Is this a normal practice for most providers/agencies ?

I usually see it requested on website screening info forms for independent providers.  I give it

Aren't you concerned with Agencies having that info ?  For what reasons would an agency need your full name ?

If my mountain if okays on P411, my verifications with Date-Check, my White Lists on TER, and my verification with RS2000 is not good enough for someone, why bother.  After all, I don't see them rushing to give me all their info and their are loads of providers that are more than fine with all the verifications I have.  

If you are jus starting out then you may have to do that or it does take longer to get established and it definitely took me longer to get my foot in the door with K-girl bookers.  But once your established as a trust worthy guy why bother.  There are loads of providers out there that won't ask for that much information if you're established.  Yeah, sure those agencies do seem to be careful with the information.  But the key words are "seem to be".  Think of the sleepless nights you might have for awhile of an LE raid shuts down one of those agencies.

for one purpose only; to keep the providers and other customers safe.  The information they want, which can vary from org to org to indie providers, is to allow them to determine that 1) you are NOT connected to LE, and 2) provide them some recourse if you are rough or abusive with a provider.  Each org has their own information requirements to accomplish this.  You are actually talking less of a risk providing your personal info to a legit org than you are an indie.  No telling what an indie might do with it if they fall on hard times and become desperate.  However, if an org uses your information in an improper way, you would expose them here and they would be finished.  For that reason, the legitimate orgs will never misuse your personal info.  Some don't even keep in on their computers once they have verified you.  I have given my work information up for verification and had the same booker ask me in casual conversation a few years later what I did for a living.  They don't care where you work.  They only care whether you work for LE.  Once they rule that out, they are usually satisfied.
