My experience is that only a few providers/agencies request this info. By and large most (at least in the SF Bay Area, and these days) settle for references from other providers; a handful go for the preferred411 or datecheck type verification.
If you're worried, perhaps you can just find someone else...
Many providers want alot of personal information,ie.email,name,occuptaion,etc. I'm worried about the privacy of my encounter. Will an upscale provider make a date without all of this information. I understand they want it for their protection. However, I do not with to release it for mine.
