There are providers who will not charge anything extra for pics/video, and there are some who will charge an extra fee.  I've seen ads or talked to some providers who will charge an extra fee anywhere from $50 to equal to their base rate(!).

A few will keep a copy of video but most will not.  One will ask you to sign an NDA.

It doesn't hurt to ask either, even if she doesn't advertise pics or videos or her reviews don't say pics or videos.  I've found a few that are ok with pics and video but they don't advertise it.  Never hurts to ask!

it isn't forbidden.  You can use p411 pages as websites in the reviews.

But let's not forget, if you have VIP here on TER, you can also search for film or photos.

I think 'going rate' is all over the map, but when offered it's usually not a super expensive thing. Probably because to the providers willing to do it at all, it's just not that big a deal.  

I've asked providers I've seen who's P-you-know-what profile says Photo And Video Friendly. I've heard  

Don't worry about it - just set up a camera
You can set up a camera but at the end I get a copy of video  
I'd like about what I charge for a trip to Greece

I think by and large the providers that allow photos/videos are  ladies with a fair amount of cam or porn experience, and the more mature women who emphasize their skills and wide menu. And of course, there are a few providers who fit into both categories
